- Multiple System Vulnerabilities: The medication dispensing cabinets were located in high-traffic areas with constant distractions, contrary to best practices. After three failed attempts to find the medication in the system, Vaught used the override function, accidentally selecting vecuronium (a paralytic) instead of Versed (a sedative) when searching for the medication.

- Normalization of Deviation: Recent hurricane-related drug shortages meant medications were frequently arriving in different forms and packaging than usual, which led Vaught to rationalize the different appearance of the medication (powder vs. liquid) as another supply change rather than a different drug.

- Absence of Safety Barriers: The medication cabinet provided no warnings about selecting a dangerous paralytic, no second verification was required, and the radiology department lacked barcode scanning technology that might have caught the error before administration.

- The Tragic Outcome: Vaught administered vecuronium, which paralyzes muscles without sedating the patient, to 75-year-old Charlene Murphy. Unlike Versed, this paralytic stops breathing while leaving the patient conscious. After Vaught left, the patient went into distress and later died. When Vaught realized her error, she immediately disclosed it to the care team.
